Short-Term Recovery with Compassion
Our Recuperative Care Services provide a safe, supportive environment for adults experiencing homelessness who need extra care after an illness or medical procedure. We help bridge the gap to independence and reduce the risk of rehospitalization.
Who Qualifies?
- Individuals aged 19+ (MinnesotaCare) or 21+ (Medical Assistance)
- Those experiencing homelessness or unstable housing
- Referred by a hospital or clinic, with short-term needs spanning up to 60 days
What We Provide
- Skilled nursing and basic health monitoring
- Wound care, medication support, and patient education
- Development of clinical and discharge goals
- Holistic care coordination, including social, behavioral, and benefit supports
Facility Comfort & Safety
- Private bed and 24/7 bathroom access
- Nutritious meals and clean environment
- Secure storage of personal belongings
- Daily wellness checks and available communication channels
Duration & Extensions
Standard services run up to 21 days. If needed, we can extend up to 60 days with proper evaluation and authorization. New diagnoses reset the 21-day limit.
Seamless Discharge Planning
We ensure safe transitions by confirming health improvement or arranging alternative care settings.
